Long Term Condition Clinics

It is important that if you have a long term condition such as Asthma, COPD, Diabetes or Epilepsy that we review you, your medication, side effects, risks and issues regularly to maintain your health.

The clinics are set up to provide you with as much time and access as you need, and they are mainly run by our experienced nurses.
The long term conditions requiring regular reviews include:

  • Asthma
  • COPD
  • Diabetes
  • High blood pressure (hypertension)
  • Epilepsy
  • Heart failure/disease
  • Rheumatoid arthritis

Please ask reception for a long term condition review with a nurse if you have not been reviewed recently or you are experiencing problems. We can arrange for a nurse who can visit you at home if you are housebound to undertake your reviews. Most conditions need reviewing annually if you are stable, although some need much more regular checks such as diabetes.
